I had a hard time deciding who would be what.  I have no logical explanation for any of this.  Derp, derp.

PPS: Maybe I should have made Lestrade ‘Command’ instead of ‘Operations’ but hell, it’s security and Scotty was a redshirt!  Gotta love the operations guys. :)

Oh, no. Irene didn’t die in books. I remember that she just left with her husband. Yeah, I just hope Moffat wouldn’t make John suffer again. :’)

Actually, although she doesn’t die during the story, Irene had died by the time that Watson wrote “A Scandal in Bohemia.” In the opening of that tale, he refers to her as “the late Irene Adler, of dubious and questionable memory.”
